  • At Sunnyhill, we are dedicated to helping people gain the skills and confidence they need to live more independently. Our goal is to support growth, independence, and strong community connections, always in line with our mission to light the path toward empowered living.

    Sunnyhill’s ISLA program is for adults (18 and older) who are diagnosed with developmental disabilities. The program helps people learn everyday life skills, build independence, and take part in community activities.

  • WHAT SUNNYHILL'S ISLA PROGRAM OFFERS:

    • One-on-one support to learn skills like budgeting, cooking, cleaning, laundry, transportation, and personal care

    • Help with making friends and getting involved in the community

    • Guidance with health-related tasks, including doctor visits, mail, and paperwork

    • Connect people with employment resources

    • Flexible service hours, up to 50 per month, based on personal goals

    • Teamwork with families, case managers, and others to promote health and well-being

PRE-Independent Supported Living Assistance (PRE-ISLA)

  • The Pre-Independent Supported Living Assistance (Pre-ISLA) program supports individuals who live with family members or other natural supports but are working toward greater independence. The program focuses on skill development and community readiness, with the goal of moving into one’s own stable housing, such as an apartment. Once participants are ready, they may transition to ISLA supports. Sunnyhill prioritizes continuity of care during this process to ensure consistent, reliable support every step of the way.

    When someone is preparing for independence, Sunnyhill provides the training, resources, and encouragement they need to succeed.

    • A three-year timeframe to transition out of the family home and into independent living

  • Sunnyhill’s Individualized Skill Development (ISD) Program has a clear focus on building essential daily living skills. This program is designed to help individuals with developmental disabilities strengthen their independence by learning and practicing the skills needed to manage everyday life. The primary goal is to support skill development, preparing individuals for greater independence and self-reliance.

    SUPPORTING EVERY STEP TOWARD INDEPENDENCE

    Sunnyhill provides the training, resources, and encouragement individuals need to build skills, gain confidence, and achieve greater independence.

  • One-on-one training in daily living skills such as budgeting, cooking, cleaning, hygiene, laundry, and transportation

    Guidance with health-related tasks, including doctor visits, mail, and paperwork

    Connections to employment resources

    Flexible service hours, up to 87 per month, based on personal goals

    Collaboration with families, case managers, and other supports to ensure continuity of care

    ISD services are available to individuals who do not live in shared living spaces such as Assisted Living (ALF) or Residential Care Facilities (RCFs).

    Participants also must not be receiving services through a Comprehensive Waiver.

    Waiver while living in Independent Supported Living (ISL) progam.

  • Sunnyhill’s Community Networking Program focuses on creating meaningful opportunities for individuals diagnosed with developmental disabilities to be included, connected, and active in their communities. This program is centered on social inclusion, recreation, personal growth, and community participation.

    The primary goal is to support individuals in building relationships, accessing opportunities, and fully experiencing community life.

    Flexible service hours, up to 108 per month, based on personal goals

    Participants must not be living in Independent Supported Living (ISL) progam.
